We are seeking a dynamic and enthusiastic Primary Care Network (PCN) Pharmacy Technician to join our team and contribute to the delivery of our vision, mission, and values. This is an exciting opportunity to become part of the highly valued and supportive Clinical Pharmacy Team within Harness South PCN, working collaboratively alongside our current PCN Pharmacy Technician.

The successful candidate will play a key role within the PCN, providing expert pharmaceutical support while participating in both local Harness initiatives and national programs to innovate care delivery and enhance patient outcomes.

Job summary

This is an excellent opportunity to be part of a rapidly expanding PCN in North West London.
The successful applicant will be joining an established and experienced PCN Clinical Pharmacy team and includes trainee pharmacists and pharmacy technicians.

Whilst the post holder will be bringing their own experience to the role, they will receive support and training in their development as follows:

Receive a robust induction process and training and support along with having a clinical supervisor.
Receive regular education and training sessions with the team provided internally and by external organisations.
Receive appropriate regular supervision via line management.
Continue to receive support whilst on the CPPE PCEP pathway and have the opportunity to be enrolled onto a university approved Independent Prescribing course.

About the Role:
We’re looking for a dedicated and compassionate Care Coordinator to join our dynamic team at the Harness Primary Care Network. In this vital role, you’ll support the delivery of person-centered care by coordinating services for patients with complex needs. Working closely with GPs, nurses, and multi-disciplinary teams, you’ll ensure seamless care, empowering patients to access the right support at the right time.

Key Responsibilities:
– Coordinate care for patients across multiple services and health professionals
– Liaise with patients, carers, and healthcare teams to ensure effective care delivery
– Assist with patient assessments, care plans, and follow-up activities
– Support patients with accessing community resources and health services
– Manage and track referrals and appointments to ensure timely care delivery
