Diabetes and Cardiovascular Disease Clinical Lead
As a Clinical Lead, you will play a pivotal role in shaping the care and management of patients with diabetes and cardiovascular diseases across our two PCNs, Harness North and Harness South. Your leadership will be crucial in driving high-quality care, leading on service transformation and quality improvement projects whilst striving to take diabetes and CVD care to the next level. This includes leading the delivery of Diabetes Wave 1 and Wave 2 Enhanced Services.
If you are passionate about improving the lives of patients with diabetes and cardiovascular diseases and are ready to take on a leadership role within our Primary Care Network, we would love to hear from you.
Salary: Salary is based on experience, skills, and clinical background, such as whether you are a nurse or a GP
