Harness Care Ltd is seeking a motivated and experienced Clinical Pharmacist to join our established and innovative team across our North Primary Care Network (PCN) in Brent. This is a full-time, permanent position working across 10 GP practices
As part of a multidisciplinary team, you will provide expert clinical pharmacy support, conduct Structured Medication Reviews (SMRs), optimise medicines use, lead on quality improvement initiatives, and contribute to the delivery of long-term condition management. You’ll play a vital role in improving patient care, reducing medication-related harm, and supporting wider PCN objectives.

Harness Care is seeking an Administrator to work within the Access Hub. The main aim of this service is to
assist our patients in accessing our services in the most effective way possible. To facilitate this the
postholder will undertake the following:
Greet and assist patients in using our enhanced access services.
Handle patient enquiries courteously, efficiently and with empathy.
Undertake administrative tasks with care and attention to detail.
Understand and work with the team to meet the key performance indicators for this role.
