Clinical Pharmacist – Harness South Primary Care Network
Location: Nominated GP Practice(s) across Harness South PCN

Salary: Dependent on experience
Hours: Full-time – 37.5 hours per week (Monday to Friday with flexibility)
Contract: Permanent
Harness Care Ltd delivers high-quality, patient-centred primary care to over 140,000 patients across Brent, working in partnership with local communities to improve health outcomes. Harness South Primary Care Network consists of 10 GP practices serving diverse populations across Neasden, Stonebridge, Harlesden, Willesden and Park Royal. We are seeking an experienced and motivated Clinical Pharmacist to join our established and innovative Clinical Pharmacy team.
In this key role, you will provide expert clinical pharmacy input as part of a multidisciplinary team, supporting population health management and the delivery of high-quality, safe and effective prescribing across the PCN. You will play a central role in medicines optimisation, structured medication reviews, long-term condition management, and reducing medicines-related risk and waste, helping to improve patient outcomes and experience.
You will work closely with GPs, pharmacy technicians, trainees, care home teams, community and hospital pharmacy colleagues, and the wider PCN team. Your responsibilities will include delivering face-to-face and remote clinics, undertaking structured medication reviews (including care home reviews), supporting repeat prescribing systems, medicines reconciliation, responding to medication queries, and contributing to audits, quality improvement initiatives, and PCN deliverables such as IIF, QOF and CQC compliance.
The role also offers opportunities for leadership, service development and education, including supervising and supporting members of the clinical pharmacy team, contributing to training, and working with Clinical Directors to shape and deliver the PCN’s strategy and vision. You will work autonomously within your scope of practice, with access to robust clinical supervision, governance structures, and ongoing professional development.
