Non-urgent advice: Internal Job advertisement – Clinical Pharmacist (Harness South)

Harness Care Ltd is seeking a motivated and experienced Clinical Pharmacist to join our established and innovative team across our North Primary Care Network (PCN) in Brent. This is a full-time, permanent position working across 10 GP practices
As part of a multidisciplinary team, you will provide expert clinical pharmacy support, conduct Structured Medication Reviews (SMRs), optimise medicines use, lead on quality improvement initiatives, and contribute to the delivery of long-term condition management. You’ll play a vital role in improving patient care, reducing medication-related harm, and supporting wider PCN objectives.

We are hiring six Harness Care Coordinator

Harness Care Ltd and Harness Primary Care Networks (North and South) provides general and extended medical services to a population of 150,000 patients residing across Brent. Harness North PCN consists of 10 GP practices based over 8 sites, caring for a population of over 75,000 patients. We work together with a range of local providers, including acute trusts, community services, social care, voluntary sector and patient participation groups, to offer more personalised, co-ordinated health and social care to our local population.
