Our Primary Care Networks work together to provide more appointments to the patients we serve in Brent.
The changes mean that practices can provide more appointments locally during the hours that best suit patients, this includes evening and weekend appointments. Patients can now book an appointment by calling our Single Point of Access on 0208 194 7355. Patients can also fill out the self-referral form below. 111 online/phone can also book patients in for local appointments in the evening and at the weekend.
Evening and weekend appointments may take place by phone, video, or face-to-face depending on clinical need. Patients may be offered an appointment at their GP practice, another local GP practice or another local NHS service.

- GP Services: (both face-to-face appointments and telephone consultations). All Acute GP services, long-term conditions, chronic reviews and medication reviews
- Nursing Services: Asthma Review, Childhood Immunisations, Smears, Immunisations and Dressing/Wound Care
- Health Care Assistant (HCA) Services: NHS Health Check, Phlebotomy, ECG, blood pressure monitoring, Immunisations, SMI Health Checks and Non-Diabetic Hyperglycaemia (NDH) Checks
- Clinical Pharmacist Services: Medication Review, Asthma, Diabetic and Hypertension Reviews
